1. Talk to your families about Manatee
2. Submit referral form
You’ll fill out a referral form that asks for basic information about the patient and their parents or guardians. Then, we’ll reach out to the family to schedule a session.
2. Assessment and intake
Once the client completes the family assessment, they will review the results with their matched therapist, discuss your family’s needs, and create their 12-week care plan together.
3. Start 12-week care plan
Our personalized care plan for families includes weekly therapy/coaching sessions and our family app that will put their hard-earned knowledge into practice.
4. Stay updated
Once families complete their intake, you'll get a clear, detailed update on their session and personalized treatment plan from their provider.
